[Commotion-admin] [luci-commotion-apps] Add application form fixes (#36)

Looks like permanent is commented out on [line 271 of apps_controller.lua](https://github.com/opentechinstitute/luci-commotion-apps/blob/master/lua/luci/controller/commotion/apps_controller.lua#L271), preventing the if statement from working on the edit page.

Incidentally, there's an expiration statement commented out on [line 239](https://github.com/opentechinstitute/luci-commotion-apps/blob/master/lua/luci/controller/commotion/apps_controller.lua#L239) as well. 

Is the fix any more involved than uncommenting line 271?
