[Commotion-admin] [commotion-docs] Add bug report form (#23)

@areynold both contact and bug report forms currently go to a special address that is then consumed by redmine. I've been researching some options. 
* php (or other script) form processor that emails to redmine, 
* make a support repo on github and link directly to the issue queue
* no contact form, just email address
