It does appear that programs at the state level are more likely to meet 
local needs.

However, in my humble opinion the fundamental problems are that

1)  there are not enough health care workers because the USA doesn't 
make enough of them,  there simply is not enough medical care available, 
and the high prices exist due to undersupply.
2)  there is not enough personal responsibility
3)  there is not enough access by individuals to the means of 
self-treatment (o no! not self treatment!)
4)  the idea of "standards of care" although having indeed much basis in 
fact, is used as a pretext for what would be theft in other contexts.
5)  Americans have bought into the lie that the delivery system as it 
exists is the only way to maintain the quality standard.
6) The health care system is too complex, too fancy, and too wasteful.

The American people seem to be as hypnotized by the 
medical-pharmaceutical-insurance industrial complex as they are by
their uric-acid bespeckled politicians and the 
military-industrial-complex.  Americans seem to be convinced that
if they dont make the necessary blood sacrifices to the Gods of 
Medicine, the Gods will become angry
and let them die.

The concept of single payer seeks to fix the problem of access to 
medical care, but it doesn't fix any
of the fundamental problems with the way that Big Med operates.
