dear media creators,

Just a reiteration of past emails on this topic: the librarians have created a 
form to make it easier for all concerned to keep the media archive up to date. 
Please fill out the forms, put your media in a manila envelope, and place in 
the box in the production room. The 2 minutes it takes to fill out said form 
will assure that your media is accessible to future media creators.
