[rfu-membership] membership spreadsheet

Attached is my plan so far for the membership spreadsheet.  I wouldn't
mail it to the list if there were names or phone numbers or email
addresses in it.

Please comment on the fields you'll need and/or like in it.

I thought about making it an acess database or a web database and
decided to 'just do it' as a spreadsheet.
