[rfu-membership] Draft Revised IMC Membership Form

I worked over the old IMC membership form to more clearly provide 
necessary info for both general IMC use and for the needs of specific 
IMC working groups.

Mostly, this includes providing options to indicate working group 
affiliations (required for RFU membership). It also provides the 
statement required for state property tax exemption, as well as a place 
for working group spokes to sign off on this option if it is requested. 
It also makes an email address mandatory to join, as well as requires a 
street address if a #4 key is requested. I believe that this meets our 
legal requirements, while still meeting the security concerns raised by 
relatively easy access to the building provided by issuance of the #4 key.

I've eliminated the credit card option that we didn't support. I also 
eliminated the Paypal option, except for Sustaining Funders. If we can 
ever get real control of the mess Paypal has been in, we may want to 
reconsider that, but until we do, I think it is best to avoid these 
complications by sticking to cash or checks as the method of payment.

There is a little elbow room for modifications. More input is solicited, 
so that the final version will meet all the needs we've identified over 
the last year.
