[rfu-membership] Draft Revised IMC Membership Form

How much room do we have to operate on here. The whole form is very
cluttered. I wonder if whatever IMC group handles these forms would
appreciate it if we cut deep and really cleaned up the form so that it
is very clear...

Luckily, I don't have the time for that.

But I did have the time to come up with a Training From. I envision
that the trainer and trainee would complete the form together, and we
would keep this with the membership form in our files. This way, we
have a way to track whether people are trained, when they were
trained, and by whom they were trained.

The training form is attached.
