[Commotion-admin] [luci-commotion-apps] Add application form fixes (#36)

Built an image and ran these tests. All behaviors functioning as expected, except the "Make Application Permanent" checkbox.

When creating an app, and checking this box, the app is saved, but the permanent checkbox is not checked when editing the app again.
