[C-U Smokefree] Restaurant Observations

Hello everyone!  As Scott mentioned, restaurant observation forms are 
now available!!
All you have to do is eat out, take the paper form with you, answer 13 
easy questions, and you've been an amazing help to the Smokefree 
movement here in C/U!!!!!
 
Currently, you can email Scott (sphays at ad.uiuc.edu) or me 
(pholze at uiuc.edu) to receive a form!!!
 
We are not just trying to cover all C/U restaurants once, but also at 
specific times during the week, classified as follows:
    Weeknight early (before 9pm)
    Weeknight late (after 9pm)
    Weekend early (Fri and Sat before 9pm)
    Weekend late (Fri and Sat after 9pm)

Feel free to cover any bar, restaurant, etc. in Champaign/Urbana that 
permits smoking!  However, the following restaurant & times have been 
covered:
    Esquire Lounge (weeknight early)
    Applebee's (weeknight early)
    Cheddar's (weeknight early)
    Dos Reales (weeknight early)
    Olive Garden (weeknight early)
    Chili's Restaurant and Bar (weeknight early)
