[rfu-membership] membership form and membership list

Well, I missed the first 30 minutes of the meeting... but the last 30
minutes of the meeting were a total disaster... I wouldn't bring
something up at the general meeting unless we had to. I think this is
something that Membership can come up on its own since the policy is
already in place, e.g. the "we have to fill out a membership form"
policy. I think, the Working group is supposed to come up with how
best to execute this policy.

FYI, I do think that I filled out two forms when I first joined a year ago.
