[rfu-membership] Forms, forms, forms...

Mike asked me to resurrect our email conversations concerning a
personnel tracking system.

Attached are the 3 forms that we left off at:
1) Training form to keep track of individual DJs
2) Show form to keep track of all shows
3) Freelance form to keep track of all showless DJs.

What does everyone think?

I think that it's a pretty robust system of forms. I just worry how
maintainable it is.
