Astrid,

Medicare for All would NOT be a " second system put in place ". It would 
make Obamacare null and void and there would no longer be subsidies from 
our tax dollars being paid to the corporate private health insurance 
industry, which would realize massive amounts of tax dollars saved in 
addition to providing EVERYONE  with healthcare.
The healthcare industry is powerful, but they are only one corporate 
special interest and hence could be defeated. Look at the tobacco 
industry. who would have thought 30 or more years ago that they could be 
regulated and curtailed as they have been in the last several decades.
You do have a valid point about " the need for the whole country being 
brought along ".
That is an ongoing tactical debate within the single payer / Medicare 
for All movement. The current strategy is to work at both the State and 
national levels. Canada was successful in their battle ( and it was a 
hard fought battle ) for medicare for All in the early 1960's by 
starting in Saskatchewan and then moving to all of the other provinces. 
Vermont had a recent successful voter referendum for a single payer 
system, but is currently blocked by a provision in the Obamacare law 
prohibiting individual States from enacting a single payer system until 
2017. Nevertheless they have a voter approved system in law and in place 
ready to go forward in 2017.
The important thing to remember is that  Medicare for All has the 
support of over 65 % of the American people, so this is an issue that 
cuts through the usual bullshit of identity politics and party politics.

The most important thing is that Medicare for All MUST be fought for. It 
is decades over due and over 50,00 Americans die needlessly every year 
and millions more ( YES Millions ) are forced into bankrupcy every year 
because of the brutal inefficient and expensive privatized healthcare 
system we currently have.
The Single payer / Medicare for All battle is the new civil rights 
movement that MUST be fought for and won.
